Liability Waiver

By signing this form, I acknowledge that I am participating in the Trunk or Treat event at my own risk. I agree that Rudyville Neighborhood Association is not liable for any injuries, damages, or losses that may occur during this event.

Code of Conduct

Participants agree to conduct themselves in a safe and respectful manner. This includes, but is not limited to, no aggressive behavior and ensuring all candy distributed is pre-packaged and safe for consumption.

Insurance

I understand that Rudyville Neighborhood Association has taken reasonable precautions to ensure a safe event, but I acknowledge that participation is voluntary and carries inherent risks.

Cancellation Clause

In the event of inclement weather or unforeseen circumstances, Rudyville Neighborhood Association reserves the right to cancel the event. Participants will be notified via the contact information provided.
